Issue is due to the junk mail list db size limit in exchange. It usually happens when a user has over 9000 contacts and “Automatically add people I e-mail to the Safe Senders List” is enabled
Resolution:
Step 1: Go to Tools->Options->Junk Email, under Safe Senders, please clear
the following two options:
Also trust e-mail from my Contacts
Automatically add people I e-mail to the Safe Senders List
Step 2:
1.Login OWA of the problem user
2.Turn off junk mail filtering
3.Turn it back on
4.Add an address to the blocked receivers list.
5.Remove the address from the blocked receivers list.
6.Please log into Outlook and check the result again.
Diamond, this worked a treat had users clicking cancel all the time this saved me loads of hassle.

Thanks! This dialog box just wouldn’t go away! It was even preventing me from adding people to my Contact List in OWA! It would just hang there for 30 secs or so and then just give a generic error.
